There is no payment form anywhere on this site, and no page here will ever ask for a card number, a bank account, or an insurance member ID. Online payment is not connected yet, so the practice will explain how to pay directly.
Anything confidential — including anything to do with billing — belongs in the private intake with a secure provider, not in a form on this website.
